Conduct a discharge assessment of each child and make them understand how it is more important to take care of them at home after discharge. 2. Conduct follow-up counseling of the child to understand how he is been taken care of in his hometown. 2. Are any takeaway activities given to the child if yes have they done that? and how it helped the child when he/ she was at home? [E] Documentation: 1. Maintain daily work on google sheets and fill forms for each child accordingly. 2. Maintain observations, Intervention tools and plan for the next session with the child. 3. Maintain Assessment- Admission, Intermediate, and Discharge of each child and generate impact report and monthly report with help of it. [F] Reporting &MIS: 1.
